The selection of an appropriate imaging sequence greatly influences the image quality and diagnostic accuracy. In this context, the Fast Spoiled Gradient Echo (3D FSPGR) sequence offers several advantages like among the other things high spatial resolution, multiplanar reconstruction capability, and a gapless acquisition feature that helps reduce partial volume effects. However, in clinical practice, comparative evaluation of image quality between 3D FSPGR sequence and conventional sequences such as Proton Density (2D PD) and Short Tau Inversion Recovery (2D STIR) in the sagittal plane still requires any further investigation. Therefore, this research was conducted to analyse the image quality of sagittal ankle joint MRI using 3D FSPGR sequence in comparison with the 2D PD and 2D STIR sequences.rnrnThis research aims to analyse and compare the image quality of sagittal ankle joint MRI using Proton Density (2D PD), Short Tau Inversion Recovery (2D STIR), and Fast Spoiled Gradient Echo (3D FSPGR) sequences. In addition, this research seeks to determine the sequence that provides the most optimal image quality based on Signal-to-Noise Ratio (SNR) values and visual anatomical assessment.rnrnThis research applied a quasi-experimental method with a descriptive quantitative approach. The research was undertaken at Radiology Department of Bhayangkara Level I Hospital of Medical and Health Centre of Indonesian National Police, in October 2025. As for the research sample, it consisted of 15 participants who met the inclusion and exclusion criteria. While the data collection was carried out through observation, experimental MRI scanning of the ankle joint using three sequence variations, measurement of Signal-to-Noise Ratio (SNR) values using ImageJ software, and questionnaire-based assessments completed by radiologists and radiographers.rnrnAs the conclusion, the results of this research showed that the sagittal Fast Spoiled Gradient Echo (3D FSPGR) sequence produced higher Signal-to-Noise Ratio (SNR) values and clearer anatomical visualization compared to Proton Density (2D PD) and Short Tau Inversion Recovery (2D STIR) sequences. Based on the quantitative analysis and subjective assessments by respondents, the 3D FSPGR sequence provided the most optimal image quality for depicting ankle joint anatomical structures.rnKeywords : MRI Ankle Joint, 3D FSPGR, Image Quality, Signal-to-Noise RatiornReferences : 42
