ANALISIS PEMERIKSAAN APEKS PARU PADA PASIEN TIDAK KOOPERATIF DENGAN KLINIS TUBERKULOSIS\r\nPARU DI INSTALASI RADIOLOGI RS X JAKARTA SELATAN
Technically, lung apex examinations undertaken at the Radiology Department of Hospital X in South Jakarta often encounter any challenges when using standard projections such as AP/PA lordotic, AP/PA axial, or apical views, particularly in non-cooperative patients. According to theory, for the patients who are weak, unstable, or unable to stand, an alternative projection namely, semiaxial AP, may be accordingly applied. However, in practical settings, even the supine position can be difficult to implement effectively. Therefore, an adapted technique referred to in this research as the semi-Fowler projection is used, wherein the patient is positioned in a semi-seated posture at a 30°–45° incline, with the X-ray tube angled 10°–20° cephalad.rnThis research aims to evaluate the procedures for lung apex examination in non-cooperative patients with clinical pulmonary tuberculosis carried out at Radiology Department of Hospital X, South Jakarta. The research was conducted starting from April up to May using a qualitative method. As for the data, they were collected through among the other things direct observation of the application of the semiaxial AP projection and the semi-Fowler projection, utilizing a Siemens X- ray machine and 35x43 cm Fujifilm CR cassettes. In addition, interviews were conducted with radiologists and radiographers. The data were then analysed by comparing theoretical concepts, practical implementation, and radiological interpretations by the radiologist, and the results were presented descriptively.rnThe results of the research indicate that both projections can be used to support the diagnosis of pulmonary tuberculosis. However, in non-cooperative patients experiencing respiratory distress, the examination technique using the semi-Fowler position proves to be more advantageous. This technique helps reduce diaphragmatic pressure and yields more diagnostically optimal images of the lung apices. This is demonstrated by the clear visualization of pulmonary structures behind the ribs or clavicles, with proper clavicular displacement, allowing for the identification of radiologic lesions such as infiltrates and cavities.
